Fibroblast growth factor 23 (FGF23) plays an important role in disorders of bone and mineral metabolism in patients with chronic kidney disease (CKD). In addition, FGF23 is thought to play a role in the pathogenesis of renal anemia. The aim of the study was to establish a relationship between FGF23 levels and erythropoietin resistance in patients in pre-dialysis and dialysis stages of CKD. Study included 80 patients (30 in pre-dialysis and 50 in dialysis of CKD). IFGF23 levels were examined in all patients and erythropoietin resistance index (ERI), age, sex, BMI, phosphate levels, vitamin B12, folic acid were calculated. The results were processed with SPSS v.20.0 for Windows using analysis of variance, variation, comparison and correlation. For significance level we assume p <0.05. The levels of iFGF23 in the pre-dialysis group were significantly lower than the levels in the dialysis group, the difference being more than twice. High levels of iFGF23 correlate strongly with the duration of dialysis treatment. A moderate proportional relationship was found between iFGF23 and ERI. The highest levels of iFGF23 are associated with the highest risk of developing erythropoietin resistance in patients on maintenance hemodialysis.
