MEDICAL SUPERVISION IN BULGARIA - EFFECT ON THE QUALITY OF CARE

Abstract

Continuous reforms in the health systems of European countries are dictated by the constant pursuit of improving the efficiency, safety and quality of health care. Specialized state bodies are being set up with the main goal of working to improve the quality and safety of medical care. The expectations of the society and the state are high, and the control bodies have the potential to be powerful modifiers of behavior. As the effects of controls can vary, the successful implementation of the chosen supervisory strategy involves several important aspects such as the competences of inspectors and the existence of clear objectives, both in terms of inspections carried out and those inspected.
The purpose of our study is to determine the effect of control on healthcare facilities and medical professionals, as well as to define possible challenges for the control institution.
In this topic we review the activities of the Executive Agency "Medical Audit" for 2019 in terms of inspections of complaints and signals related to adverse (lethal) outcome and compare the results with those of 2017 and 2018. In view of the results we discuss the degree of impact of medical control on the controlled facilities, as well as the challenges facing the state authority.
Conclusions Each country has its own historical and cultural context, which creates an unique environment for regulation and supervision. The outcome of the study requires careful interpretation, as the lack of statistical significance may not mean that there is no effect on the quality of care or that the effect is unsatisfactory. The effects of control on a number of outcomes, including those related to patients, healthcare professionals and healthcare facilities, need to be taken into account.
