Le Fort fractures are severe craniofacial injuries that involve disruption of the midface and require precise diagnosis and timely surgical intervention. The basic principle of treatment is anatomical repositioning and stable fixation in order to restore function and aesthetics. Le Fort osteosynthesis is the gold standard in the surgical treatment of midface fractures, providing stable reconstruction, anatomical restoration of facial contours and optimal function provided that the treatment is timely and professionally performed. The aim of this specialist paper is to present the modern principles and techniques of osteosynthesis in Le Fort fractures, to analyze the results of the applied methods in patients and to identify factors that influence the occurrence of complications. According to the conducted analyses (radiological, clinical, type and location of the fracture) in patients surgically treated with Le Fort fractures, the applied osteosynthesis techniques with miniplates and titanium screws, 3D plates, have obtained excellent functional and aesthetic outcomes. In the future, further development of 3D planning, individually manufactured implants and minimally invasive techniques has the potential to further improve the precision and safety of osteosynthesis in Le Fort fractures. In terms of intraoperative and postoperative complications, the outcomes are also very good. In most patients, satisfactory anatomical repositioning and stable occlusion with minimal functional disorders were achieved. The most common complications were recorded in cases most often related to smoking, poor oral hygiene, delayed treatment, etc. Radiological evaluation showed satisfactory consolidation of the fractures in most patients. Osteosynthesis with modern titanium systems provides reliable and stable fixation in Le Fort fractures with good functional and aesthetic results when the principles of precise preoperative planning, accurate repositioning are applied. With further development of technology and accumulation of clinical experience, further reduction of complications and further improvement of the quality of treatment can be expected. Of course, timely diagnosis, multidisciplinary approach and patient monitoring are key to reducing complications and optimizing the outcome,
which significantly improves the quality of life of patients.
