Abstract

Immediate implant placement and loading can offer several clinical advantages, particularly in the esthetic zone, including preservation of the alveolar ridge architecture and reduction of soft-tissue collapse following tooth extraction. However, careful case selection and adequate primary implant stability are essential for achieving predictable clinical outcomes.
A 45-year-old male patient presented with a horizontal fracture in the middle third of the root of the maxillary left central incisor. Following atraumatic extraction, an immediate dental implant was placed. Primary implant stability of 50 Ncm was achieved. Following the surgical procedure, digital intraoral scanning was performed, and a definitive screw-retained zirconia crown was fabricated using a CAD/CAM workflow. The crown was subsequently delivered in infraocclusion to minimize functional loading during the initial healing period.
The immediate implant placement and loading protocol provided a favorable esthetic and functional outcome while avoiding some of the limitations associated with conventional provisional prosthetic solutions. The digital workflow facilitated the fabrication and delivery of the definitive screw-retained zirconia crown.
Immediate implant placement and loading in the anterior frontal zone may provide predictable and favorable clinical outcomes when appropriate case selection is combined with adequate primary implant stability and careful postoperative management. The use of a screw-retained zirconia crown within a digital CAD/CAM workflow may provide satisfactory esthetic and functional rehabilitation of the anterior region
