ATOSIBAN IN PREVENTION OF PREMATURE BIRTH - CLINICAL ASPECTS

Abstract

Preterm birth is a leading cause of neonatal morbidity and mortality. Tocolytic therapy plays a key role in delaying delivery, in order to allow the use of corticosteroids and improve neonatal outcome. Atosiban, as a selective antagonist of oxytocin receptors, is used as one of the modern tocolytics with a favorable safety profile.
Objective: The paper aims to analyze the clinical application and outcomes of the use of atosiban in the prevention of preterm birth in patients treated in peripartum intensive care.
Materials and methods: This paper presents a retrospective analysis of patients treated with atosiban over a two-year period, in the peripartum intensive care unit of the PHI “GAK”-Skopje. The number of doses administered, gestational week at admission, type of pregnancy and pregnancy outcome were analyzed.
Results: Over a two-year period, 192 patients were treated with atosiban. The most common gestational week for administration was 32 and 33 weeks, respectively. Singleton pregnancies predominated over twin pregnancies. According to the outcome, patients were discharged from the hospital, and according to the mode of delivery, cesarean section was more common than spontaneous delivery.
Conclusion: Atosiban shows significant efficacy in delaying preterm birth, especially when applied in early treatment protocols and at an optimal gestational period. Its use allows stabilization of a significant number of patients and potentially improves perinatal outcome. Further studies are needed for a more detailed evaluation of long-term effects and comparison with other tocolytic agents
